All exams must be taken at the designated date and time. Generally, no make-up exams will be given.

D. Module Examination Policy

To aid in the retention of past information, approximately 10% of every examination may be based upon previous Respiratory Care course materials. This policy may apply to any RESPCARE prefixed courses.

E. Test Regrading Policy

COURSES: This policy applies to the following courses only: RESPCARE 203, RESPCARE 223, RESPCARE 303, RESPCARE 323, HLTHST 216 and others as announced. IT DOES NOT APPLY TO FINAL EXAMINATIONS.

PURPOSE: The educational justification for grading is to assist students in the complex process of intellectual self-evaluation. If a grade is to be intellectually useful to a student, then he or she must understand the reason for it. Commonly, the method used to communicate the "correct" answer is for the instructor to provide it while correcting the exam. While we as faculty in the RC program encourage students to conference with us after receiving an exam, frequently students believe that the grade and our corrections of the exam are all the communication that is needed. Therefore, errors may be repeated, students become less responsible for the information and the burden of providing the correct answer falls exclusively on the instructor and the student doesn't learn the information. In an attempt to make students more responsible for information contained on tests, and to provide some reward for this effort, the following policy has been established.

METHOD:

1. Instructors will be responsible for scoring exams. Instructors will not make corrections or indicate the correct response.

2. After having an exam returned, students will have one week to make any corrections. (The instructor is not responsible for providing reminders.)

3. All corrections must be made on a paper other than the exam.

4. If your exam is returned on or before the one week deadline, the instructor will evaluate the corrections made.

5. After having evaluated your corrections, the instructor will add the following percentage points to your grade for the exam:

a. Zero (0) percentage points will be added if you make no corrections or correct less than one half of the total number of missed items.

b. Three (3) percentage points will be added if you revise CORRECTLY at least one half of the missed items.

c. Five (5) percentage points will be added if you revise CORRECTLY all missed items.

6. For multiple choice and true false questions, indicate the correct answer and briefly explain the answer.

7. Examples of corrections may include correcting math errors, reworking calculations, adding additional items to responses, labeling and/or correcting graphs, answering questions you left unanswered on the exam, etc.

* 8. In general, your test re-grade should show evidence of learning. You may wish to document page numbers or lecture note dates where you found the information.

F. Attendance and Lateness Policy

1. Attendance

a. Theory and Laboratory - Attendance is not mandatory for RESPCARE 203, 204, 223, 224, 303, 304, 323, or 324. This means that the grade will not be reduced if the student is absent; however, attendance is critical and if a student is chronically absent from these courses the student may be counseled by the faculty or Department Chair.

b. Attendance is required in all other RESPCARE and HLTHST prefixed courses.

2. Lateness:

Each student is expected to be in class on time. Students arriving after the scheduled starting time are disruptive to class. Please be considerate of the other students and the faculty and be in class on time.

G. Written Assignments

Written assignments will be considered late if handed in after 5:00 pm, or other designated time, on the day due. The final grade for that paper may be reduced 10 % for each school day late. All papers must be handed in, regardless of lateness, to pass the course.

Papers and other assigned writings must be thoroughly proofread for spelling and grammatical errors before they are submitted for grading. Please be aware that computerized spell checking is very helpful, however it is not a substitute for proofreading. We encourage students to exchange papers with each other so that "fresh eyes" are able to identify problems before they are to be graded. Papers that are found to have significant errors in spelling and grammar will not be graded, rather they will be returned to the student for corrections and resubmission. The delay between the original due date and the resubmission date will be treated the same as a late paper, and will be penalized 10% for each school day late. This policy will not apply to exams or quizzes. Students are encouraged to utilize the services of the Writing Center for assistance with writing skills. It is located in Liberal Arts, Room 200. The phone number is 426-1298.

H. Basic Life Support Certification

All students will be required to attend and successfully complete a Basic Life Support (BLS) course as provided by the faculty. This course will be held in the fall semester of the sophomore year. If the student has a current Health Care Provider BLS card that is valid through the junior spring semester, they are not required to attend this workshop.

I. Advanced Cardiac Life Support

All students will attend an Advanced Cardiac Life Support (ACLS) workshop presented in the spring semester of the junior year. It is important that students prepare for and successfully complete the ACLS program; most hospitals now require ACLS certification of Advanced Respiratory Care Practitioners.

J. Academic Dishonesty

"Cheating or plagiarism in any form is unacceptable. The University functions to promote the cognitive and psychosocial development of all students. Therefore, all work submitted by a student must represent her/his own ideas, concepts, and current understanding. Academic dishonesty also includes submitting substantial portions of the same academic course work to more than one course for credit without prior permission of the instructor(s)." BSU Student Code of Conduct, Section 1. http://www2.boisestate.edu/studentconduct/Student%Code%20of%20conduct.htm

K. Competency and Attitudinal Policy

Any student may be temporarily suspended, and eventually dismissed, from the program for conduct involving their attitude or competency that jeopardizes patient care or safety, or for performance that is unsatisfactory, or whose personal character prevents desirable relationships within the health care facility or the Respiratory Care Program. Persons having the authority to temporarily suspend a student are BSU Respiratory Care instructional staff, part-time clinical instructors with University appointments, and any hospital Respiratory Care Department Director, or the equivalent. A student may not be removed from the program without written notification by the Department Chair.

In the event of suspension, written notification of the circumstances must be furnished to the student within 36 hours of the circumstances.

In the event the Department Chair decides it is necessary to permanently remove a student from any course or from the program, the College of Health Sciences' Policy and Procedure for the Suspension or Dismissal of Students because of Unsafe, Unethical or other Unprofessional Practices in Clinical/Internship Patient Care Settings will be followed. A copy of this policy is available in the Department.

L. Counseling

Faculty are available to help, when feasible, any students having academic difficulties. Students perceiving that their academic needs are not being met by this process are encouraged to see the Department Chair during the semester in which the difficulties arise. The University Counseling and Testing Center (426-1602) offers services designed to help the student with both academic and personal difficulties. In addition, the Student Support Program (426-3583) offers information and services that may be useful to the student.

M. Advanced Standing Policy

Admission to the Junior Year of the Respiratory Care Program:

Student Waiver of Student Health Insurance
Introduction:

Neither the Department of Respiratory Care nor its associated clinical affiliates carry health insurance for students. A student health insurance policy is included in the BSU fees. It is to be used for emergencies, injury, and trauma.

If you are injured in your assigned clinical site, go to the Student Health Center for treatment. For after-hours situations, you may go to your personal physician or to an emergency care center. You are responsible for submitting the claim, for any deductible, and any other expense associated with the injury. You are not an employee at your clinical site and they will not "write-off or forgive" your treatment costs.

If you have private health insurance, you may ask for a refund of the insurance portion of your student fees. Please see the current BSU Catalog for details and deadlines.

Regardless of your insurance coverage, all full-time students have access to the Student Health Center. See your Student Handbook for an explanation of these services.

Policy:

All students enrolled and participating in a clinical course through the Department of Respiratory Care MUST be covered by a health insurance policy.

Hepatitis B Vaccination Choice Form

It is the recommendation of the Department of Respiratory Care, that all students receive the Hepatitis B vaccine prior to entering clinical areas. This recommendation is for the protection of students who will be exposed to blood and blood tinged fluids during their clinical rotations. Contracting an active case of Hepatitis B will endanger patients who the student might contact, and that student will be removed from patient contact until such time that they are judged to no longer be contagious. This condition may also affect employment in medical care after graduation until Hepatitis B titers return to acceptable levels.

With the above information in mind, I choose the following:

1. I choose to receive the vaccine (given in three doses) and will provide the BSU Respiratory Care Department with written verification of each dose within one week of vaccinations.

Signature__________________________________________ Date______________

2. I choose not to take the Hepatitis B vaccinations. I understand that the vaccinations are a preventative measure in case I might become exposed to the Hepatitis B virus during my clinical experiences at Boise State University. I accept responsibility for not choosing to be vaccinated.

3. I have completed the series of 3 Hepatitis B vaccinations.
